And most standard panels only ask one or two questions when the real picture needs four or five.

The TSH-Only Thyroid Panel

TSH measures how hard your pituitary gland is working to get your thyroid to respond, not how much thyroid hormone is actually reaching your cells. According to the American Thyroid Association, a full picture of thyroid function includes free T4, the usable hormone circulating in your blood, and often free T3, which is the active hormone your cells actually use.

A woman can have a textbook-normal TSH and still be converting T4 to T3 poorly, which means her cells aren't getting the active hormone even though her pituitary and total thyroid hormone numbers look fine.
